The first step is to call or come by our clinic to let us know you're interested. You can also fill out the contact form on our home page. We will help facilitate a referral with your primary care provider and verify your insurance. You will then be invited to attend an intake assessment, during which you will get a chance to meet our clinician and learn more about our program(s). If you decide to participate in the program, then we will complete the assessment to ensure the program is a good fit for you. The intake assessment is a no-obligation appointment; it is informational and you don't have to commit to participating in any of the programs unless you choose to.
The three programs are formatted similarly. The pain management and stress management programs consist of once a week classes for 10 consecutive weeks. The tobacco reduction program consists of once a week classes for 7 consecutive weeks. The programs are primarily behavioral and movement based, and utilize techniques that are proven to be effective. No medications are prescribed by the clinic. All medication management continues to be provided by the client's Primary Care Provider.
The program curriculum includes educational, experiential and body movement components to best assist the client to improve their health and quality of life. The body movement component consists of gentle yoga techniques led by an experienced and compassionate instructor. Relaxation training, stress reduction, and mindfulness are other areas that are focused on in the classes.
The programs are presented in a small group class setting as it gives participants an opportunity to share experiences with those who have similar struggles. The information and experiences are presented in an upbeat and engaging manner which has resulted in a 95% highest approval rating from program graduates.

Insurance: Revitalize Wellness Center is contracted with the Oregon Health Plan with no fees or co-pays. Some Medicare and private insurance plans are accepted as well - please call us if you have a question about your insurance coverage for our programs.
Some Oregon Health Plan members have a benefit for medical transportation or mileage reimbursement to help you get to your appointments; please let us know if you need help with this.
